Job Summary

The Home Therapy Registered Nurse is a key member of our team, responsible for providing patient training and ongoing support for those choosing home dialysis. This role requires a high level of clinical expertise and excellent communication skills to deliver superior customer service.

Key Responsibilities

• Train patients and their care partners in self-care peritoneal dialysis or hemodialysis, ensuring compliance with federal and state regulations.
• Conduct home visits to assess the patient's environment and evaluate their performance, with a focus on improving care.
• Communicate patient assessment results and nursing interventions to physicians, team members, and other stakeholders.
• Document all nursing services in the Electronic Medical Record, ensuring accurate and timely documentation.
• Participate in infection control monitoring and implementation, and record results as requested.
• Be familiar with emergency equipment and procedures, and communicate regularly with patients on emergency preparedness.
• Ensure clinic and patient supply inventory is appropriate for patient prescriptions and place orders in a timely manner.
• Continuously seek to improve patient and quality outcomes through tracking and trending of program and patient performance.
  
  Requirements
  
  To succeed in this role, you will need:
  
  
  • A current RN license in the applicable state, with a focus on maintaining current and good standing.
  • 12 months of clinical experience in providing nursing care, with 3 months of experience in the specific modality for which you will provide home dialysis training.
  • Previous experience in providing nursing care to patients on maintenance dialysis is preferred.
  • CPR certification is required prior to patient care assignment.
  • Confirmation of ability to distinguish all primary colors.
  • Must successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency.
  • Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
  • Basic computer skills are required, with proficiency in all USRC clinical applications within 90 days of hire.
